2024-2025 Enrollment Form

We currently have openings in our 4-year-old and PreK classes. Open classroom spots are filled on a first come, first serve basis based on receipt of the enrollment form AND fee. You will receive an email confirmation once your enrollment form AND fee has been received and your child has been placed on the classroom roster. Once the classes are full, a waiting list will be generated based on receipt of the enrollment form. 

Our 1-year-old, 2-year-old and 3-year-old classes are full. We are working on our wait list for these classes. Class waiting lists are very important. Wait lists are generated on a first come, first serve basis, according to when the enrollment forms are received. If you would like to place your 1-year-old, 2-year-old or 3-year-old on our waiting list, please fill out the enrollment form only. **Do not submit the fee at this time.

    Lunch Bunch Option:
    Lunch Bunch is offered to our 3s, 4s and PreK classes. Children bring a "nut free" lunch and dismiss at 12:30.
    Enrichment Class Option:
    The enrichment class is for our 4s and PreK classes only. The enrichment class is from 12:30-2:30.
    Family Information:
Please note that OSP requires children to be up to date on all vaccinations. Please contact the school for questions about the requirements.
Non-refundable $125 Enrollment Fee is due with completed Enrollment Form to secure placement in the program.  **Do NOT submit the fee for a class showing full.
